ANIMAL ENTRY PERMIT PROCESS AND REQUIREMENTS
Submission Timeline
• To allow adequate time for review and processing, all required import documents must be submitted at least 30 days prior to the intended arrival date.
• Documents submitted less than 14 days prior to entry cannot be guaranteed to be processed in time and may result in prolonged quarantine or denial of entry.

 

Required Documentation

USA ARRIVAL

1. All pets arriving from USA origin (including territories) must submit the following documentation:
   1. Application for Entry Permit (use our most updated form only) Click here for form.
   2. Current and Prior Rabies Certificates – both must have the following information:
      1. Veterinarian signature
      2. Rabies vaccine manufacturer
      3. Serial or Lot number for the vaccine
      4. Serial or Lot expiration date
      5. Date the vaccine was administered
      6. Whether it is a 1-year or 3-year vaccination
      7. For Current Rabies Certificate ONLY: microchip identification number (microchip must have been administered before current rabies vaccination)

• Vaccination Records for the following vaccines:
  1. For Dogs:
     1. Distemper/parvo/parainfluenza/hepatitis vaccination (1-year or 3-year vaccination)
     2. Leptospirosis vaccination (1-year vaccination)
     3. Bordetella vaccination (1-year vaccination)
  2. For Cats:
     1. Rhinotracheitis/calicivirus/panleukopenia vaccination (1-year or 3-year vaccination)
  3. Flight itinerary: must include pet’s itinerary from original departure state and all connecting flights
  4. Quarantine Facility Reservation 
  5. FAVN Tests Results
     1. Results mustbe sent straight from a CDC approved testing facility via email (preferred) or mail (https://www.cdc.gov/importation/dogs/approved-labs.html)
     2. Microchip identification number must be included on the test results

• Payment of $65
  1. $60 for Entry Permit
  2. $5 for annual Pet License
  3. Payment can be mailed or paid in person to/at 163 Dairy Rd., Mangilao, Guam 96913. We accept cash (exact amount), cashier’s check, money order, or local bank check. A representative may also make payment on their behalf.
  4. Checks made out to: Treasurer of Guam
• Health Certificate done by a USDA accredited veterinarian within 10 days of travel.
  1. Microchip identification number must be included
  2. Please note, if HC is not done by a USDA accredited veterinarian, their quarantine status will change.

1. If all required documentation is submitted, pets arriving from the US, including territories, will qualify for the 5 days or less commercial quarantine program. Upon arrival, the pet will enter quarantine until a post-arrival veterinary examination is performed and the microchip number is confirmed. The pet may then complete the remainder of the 120-day quarantine period in home quarantine.
2. IMPORTANT* If pet has an international connecting flight, they must remain within the airport or designated quarantine facility. If the pet leaves the airport, it will be considered an international arrival and will be subject to those set of requirements and quarantine rules.

 

INTERNATIONAL (NON-EXEMPT FOREIGN) ARRIVAL 

1. All pets arriving from international origin (excluding Australia, British Isles, Hawaii, Japan, and New Zealand–see EXEMPT for these areas) must submit the following documentation:

   2. CDC-registered Animal Care Facility Reservation

      2. Foreign-vaccinated dogs that have been in a high-risk country for dog rabies within the last 6 months must have a reservation at a CDC-registered animal care facility before entering the U.S. Contact the facility well in advance of your scheduled travel to make a reservation for the required exam and revaccination against rabies. If you can’t get a rabies serology titer result for your dog before it arrives in the United States, you must also make a reservation for a 28-day quarantine after revaccination. Your dog must arrive at the U.S. airport associated with the facility where you have made a reservation.
   3. International arrivals do not qualify for home quarantine. Therefore, pets must wait the full 120 days after the FAVN sample was received at the CDC-approved lab facility before arrival to Guam, or remain in a commercial quarantine facility on Guam until the 120^th The exception to this rule is if the pet was screened by a military veterinarian prior to arrival to Guam.

 

EXEMPT ARRIVAL

1. All pets arriving from Australia, British Isles, Hawaii, Japan or New Zealand must submit the following documentation:

      2. Rabies vaccine manufacturer

      6. Whether it is a 1-year or 3-year vaccination
      7. Microchip identification number (microchip must have been administered before current rabies vaccination)
   3. Vaccination Records for the following vaccines:
      1. For Dogs:
         1. Distemper/parvo/parainfluenza/hepatitis vaccination (1-year or 3-year vaccination)
         2. Leptospirosis vaccination (1-year vaccination)
         3. Bordetella vaccination (1-year vaccination)
      2. For Cats:

• Flight itinerary: must include pet’s itinerary from original departure country and all connecting flights

1. FAVN Tests Results or Affidavit for Export
   1. For FAVN test results:
      1. Results mustbe sent straight from a CDC approved testing facility via email (preferred) or mail (https://www.cdc.gov/importation/dogs/approved-labs.html)
      2. Microchip identification number must be included on the test results
   2. For Affidavit for Export: must state the pet resided in country for a minimum of 120 days prior to immediate export to Guam. Typically, this statement is included on the 10-day health certificate.

3. Health Certificate done within 10 days of travel
   1. Microchip identification number must be included
   2. Must be in English or English translated. Please keep original language visible for verification of translation.

• Confirmation appointment with a veterinary clinic within 72-hours of arrival on Guam
• Import CDC Form (for dogs only)(not needed for Hawaii):

1. If all requirements are met, pets arriving an exempt area are not required to quarantine, however, within 72 hours of arrival, the owner must take the pet to one of the private practice veterinarians for a post-arrival examination and the results of the examination sent to our office.
2. IMPORTANT* If pet has an international connecting flight (from non-exempt foreign country), they must remain within the airport or designated quarantine facility. If the pet leaves the airport, it will be considered an international arrival and will be subject to those set of requirements and quarantine rules.

 

TRANSITS

1. All pets arriving to Guam but have a connecting flight to depart must still submit the following documentation:
   1. Application for Entry Permit (use our most updated form only) Click here for form – please note that even though the pet is a transit, they are still entering Guam, therefore, must have an Entry Permit.
   2. Current Rabies Certificates – must have the following information:
      1. Veterinarian signature
      2. Rabies vaccine manufacturer
      3. Serial or Lot number for the vaccine
      4. Serial or Lot expiration date
      5. Date the vaccine was administered
      6. Whether it is a 1-year or 3-year vaccination
      7. Microchip identification number (microchip must have been administered before current rabies vaccination)
   3. Vaccination Records for the following vaccines:
      1. For Dogs:
         1. Distemper/parvo/parainfluenza/hepatitis vaccination (1-year or 3-year vaccination)
         2. Leptospirosis vaccination (1-year vaccination)
         3. Bordetella vaccination (1-year vaccination)
      2. For Cats:

• Quarantine Facility Reservation – in the event there is a long layover or a flight gets delayed, this will be needed

CERTIFIED SERVICE ANIMALS (Dogs only)

1. Service animals are exempt from quarantine so long as they meet all other requirements for entry (following above sections), including the post-arrival veterinary exam. There are two categories for service animals and both differ slightly. We will review the application and required documents to determine post-arrival process.
