ANIMAL ENTRY PERMIT PROCESS AND REQUIREMENTS
Submission Timeline
• To allow adequate time for review and processing, all required import documents must be submitted at least 30 days prior to the intended arrival date.
• Documents submitted less than 14 days prior to entry cannot be guaranteed to be processed in time and may result in prolonged quarantine or denial of entry.
Required Documentation
The following documents must be submitted as part of the permit application packet:
• Completed Application for Entry Permit
• Vaccination records
• Rabies certificates
• FAVN test results (for non-exempt areas)
• Flight itinerary
• Quarantine reservation confirmation
• Payment of applicable entry permit fees ($ 65.00/pet)

• Canine International arrivals also require:
- 1. CDC Import Form
2. CDC Foreign Rabies & Microchip Form for high-risk rabies areas
• The Health Certificate, signed within 10 days of travel, should be the final document submitted
